■ Terrier breed needs haircut & hasn’t been groomed in last 5 months
■ Doodle breed needs haircut & hasn’t been groomed in last 4 weeks
■ Non-shedding breed needs haircut & hasn’t been groomed in last 3 months
2. Special Behavior Waiver
■ Scared of scissors or clippers
■ Biting, nipping, or aggressive
■ Hates grooming
■ Hates paws or feet touched
■ Doesn’t like nails clipped
■ Wiggly / won’t stay still
■ May nip
■ Mentions prescriptions, meds, or drugs
■ Doesn’t like face touched but needs haircut
3. Medical 1.0
■ Dog or cat is 12 years old or older
4. Medical 2.0
■ Any medical condition of any kind mentioned
5. Deskunk Waiver
■ Dog or cat sprayed by skunk
6. Flea/Tick Waiver
■ Mentions fleas
■ Requests flea shampoo
